How to Customize

Once you purchase these videos, you must email us with the following:

• Your name as you want it to appear in the video
• Your office name and address
• A headshot photo of yourself in order to create an animated rendering (jpg file)
• Your office logo (if you have one) (jpg file)

For Booklet Customization:

• Front Cover - Photos, Office Information (name, address, phone number, web address, logo, etc)
• Inside Front Cover - (suggested) Doctor/Staff photo, Personal Letter to Patient (You are welcome to use the sample letter seen here)
• Inside Back Cover - (suggested) Patient Testimonials
• Back Cover - Office logo (if you have one)

Once editing is complete, the videos and the PDF file for the booklets will be emailed to you.  Please allow 2-3 weeks upon receipt of above information.

No refunds, no returns.  Orders may be cancelled prior to production.
